		localized infection:	>0.5 mcg/mL
		systemic infection:	>1.0 mcg/mL
	Hydroxyitraconazole
		No therapeutic range established; activity and serum concentration are similar to parent drug.
Critical Values:	Itraconazole+Hydroxyitraconazole (combined total): >or= 10.0 mcg/mL *Every critical result will be called back*
Limitations:	Enteropathy, H2-histamine receptor blockers, hepatic enzyme inducers, and other variables can result in low to nondetectable serum levels with concomitant high risk of therapeutic failure. Assay range = 0.1-10.0 mcg/mL
Methodology:	Liquid Chromatography-Tandem Mass Spectrometry (LC-MS/MS)
