SPECIMEN REQUIREMENTS
Specimen Type:
Urine, 24Hr collection

Preferred/Acceptable:
Brown, 24 hour urine collection jug with preservative added prior to start of collection. Call lab at 314-454-4268 to obtain collection jug with preservative.
 
Collection Volume:
Entire specimen collected over a 24-hour period.
 
Patient Preparation:
For 48 hours prior to, as well as during the collection, the patient should: limit the following to 1 serving per day: 
  - Fruits
  - Vegetables
  - Nuts
  - Caffeinated beverages or foods.
If clinically feasible, discontinue the following medications at least 48 hours before specimen collection:
  - Acetaminophen (Tylenol or generic versions)
  - Aspirin
  - Antihistamines
  - Cough syrups
  - Cold and flu medications
Patient should also avoid caffeinated beverages such as tea and coffee or caffeinated foods for 48 hours before and during specimen collection.
 
Sample Rejection:
Specimens other than 24 hour collection.
Mislabeled or unlabeled specimens.

Refrigerate specimen during collection and send to lab promptly at end of collection period.
 
3.

Limitations:
Intake of food with a high content of serotonin (avocados, dates, eggplant, all fruit [including bananas, cantaloupe, grapefruit, kiwifruit, melons, pineapple, plantains, plums], all nuts [including hickory nuts, butternuts, pecans, walnuts], and tomatoes and tomato products) within 48 hours of the urine collection could result in falsely elevated 5-hydroxyindoleacetic acid (5-HIAA) excretion.
Numerous drugs affect the excretion of 5-HIAA by different mechanisms, including increased serotonin synthesis, metabolism, and release and inhibition of uptake. The following medications can interfere with 5-HIAA results. If clinically feasible, discontinue the following medications at least 48 hours before specimen collection. Acetaminophen (Tylenol or generic versions), Aspirin, Antihistamines, Cough syrups, Cold and flu medications

Lab Processing instructions:
Pre-collection: Add 25 mL of 50% acetic acid, as preservative, to collection container to be used. Label jug as containing preservative (acid). 
Use 15 mL of 50% acetic acid for children <5 years old.
This preservative is intended to achieve a pH of between approximately 2.0 and 4.0.
Post-collection: When specimen received in lab, mix sample well and measure total volume collected. Aliquot urine into a plastic, 13-mL urine tube. 
Note total volume collected on aliquot container and in cerner.

5.0 mL (minimum 1.0 mL) urine.
